A More Thoughtful Approach to Your Health

Port Primary Care & Wellness was created around a simple belief: healthcare works better when your provider has the time to know you.

Another Option for Your Everyday Care

For many patients, the traditional healthcare system simply isn’t working the way it should.

Rising deductibles and out-of-pocket expenses have made the cost of care increasingly difficult to predict. At the same time, busy medical offices often leave providers with limited time to spend with each patient.

Port Primary Care & Wellness offers another option. Our approach combines the familiarity and continuity of traditional primary care with some of the accessibility, personalization, and transparency often associated with concierge medicine.

Patients receive care from the same provider at each visit, with appointments that allow more time to talk through concerns, ask questions, and develop a plan together.

Our goal isn’t to add more healthcare to your life. It’s to make the healthcare you receive feel more useful, understandable, and personal.

Board-Certified Family Nurse Practitioner

Sara Castantini is a board-certified Family Nurse Practitioner with more than a decade of experience caring for patients.

Sara began her nursing career after graduating from Clemson University with a Bachelor of Science in Nursing in 2010. She spent five years as a bedside nurse at Portsmouth Regional Hospital, working on the medical/oncology and interventional cardiology units.

Wanting to expand her role in helping patients manage their health, Sara went on to earn her Master of Science in Nursing from Massachusetts College of Pharmacy and Health Sciences in 2015 and became a Family Nurse Practitioner.

Throughout her career, Sara has developed a strong interest in integrative, patient-centered medicine and in looking beyond individual symptoms to better understand their underlying causes.

She has completed additional education through the Institute for Functional Medicine’s Applying Functional Medicine in Clinical Practice program and is continuing advanced education focused on the treatment of perimenopause and menopause.

For Sara, starting Port Primary Care & Wellness is an opportunity to practice medicine differently — to spend more time with patients, build long-term relationships, and create individualized care plans that reflect each person’s needs and goals.
